Michael Forman

LYMAN-Michael Hugh Forman, 64, passed away Monday November 28, 2016 in Evanston.

Funeral services will 1:00 PM Friday, December 2, 2016 at the Lyman LDS Church with a viewing starting at 12:00 PM. Interment will be in the Lyman Cemetery.

Mr. Forman was born in Evanston April 21, 1952 to Howard and Margaret Richins Forman and he was a security guard for FMC in Green River for over 45 years. He married Mayla Rollins in 1973 in Evanston.  He enjoyed watching football, competing in gambling tournaments in Las Vegas and watching westerns especially with John Wayne as the star.
Mr. Forman was preceded in death by his father; grandmother, Laura Forman; father in law, Everett Rollins; mother in law, Ruth Rollins and a brother in law, Richard Rollins.

Mr. Forman is survived by his wife of 44 years, Mayla Forman; two daughters, Shandi Cunningham and her husband Tommy of West Jordan, UT and Arianne Forman and her fiancé Larry Gomez of Lyman; three grandchildren, Landen Cunningham, Nevaeh Kofford and Kamrielle Cunningham; his mother, Margaret Roitz of Ft. Bridger and a brother, Marty Forman of Texas.

Pallbearers will be Marty Forman, Thomas Cunningham, Larry Gomez, Jim Morretti, Glenn Barker, Lance Gilbertson, Mike Shassetz and Dean Johnson. Honorary Pallbearers will be Ricky Hoopes, Dave Black, Brad Francis, Harry Harrison, Jim Eckly, John Comfort, Bob Charles, Marty Walker, Ramon Walker and Tim Slagowski.
